Membership Application

	FULL MEMBERSHIP - Annual dues $175 Any publicly funded school or representative employee of that school engaged directly in providing truck driver training shall be eligible for FULL membership in this Association. Each Full Member of the Association shall have one vote and is eligible for a position on the Board of Directors.
	ASSOCIATE MEMBERSHIP - Annual dues $350 Any industry or representative employee of that industry which indirectly supports public truck driving schools shall also be eligible for ASSOCIATE membership. Associate members will be limited to agencies such as trucking companies, insurance companies, publishers, and equipment manufacturers interested in the welfare of publicly funded truck driving schools. Associate members are non-voting and ineligible for the Board of Directors. Note: If applying for Associate Membership, briefly answer the following questions as an attachment. 1) State the nature of your company's relationship to the truck driving industry. 2) Explain why your company feels it should be a member of NAFPTDS.
	AFFILIATE MEMBERSHIP - Annual dues $250 Administrators or staff at a publicly funded school interested in the trucking industry, or administrators in schools that contract with proprietary organizations to provide truck driver training. Affiliated members are non-voting and ineligible for the Board of Directors. Note: If applying as a school that contracts with proprietary organizations, provide the name of the organization providing truck driver training:

DUES SCHEDULE

Bylaws National Association Of Publicly Funded Truck Driving Schools

ARTICLE IV - DUES AND FEES
Section 1. Dues for all membership categories in this Association shall be fixed from time to time by resolutions of the Board of Directors adopted at any regular meeting of the Board of Directors, ten (10) days prior to the meeting. All Directors should receive written notice of the resolution. Dues are $150.00 per year for Full Members, $300.00 per year for Associate or $225.00 for Affiliate Members.

Section 2. Annual dues shall be payable to the Treasurer on January 1st of each membership year. Initial dues will be payable according to a prorated schedule at the time of application.
